Thanks for writing this, Bill. I wanted to comment on one thing in particular:

I'd argue that the problem isn't a *lack* of accountability; it's *accountability applied to the wrong things*. There's a ton of accountability today for someone who hurts the wrong person's feelings (intentionally or otherwise) and little or no accountability for those who squander institutions and assets with which they've been entrusted.
